New Patient Registration


If you have moved to the area and wish to register with the practice, call and we will give you information on the practice area and the services offered. We will require your NHS number, proof of ID and proof of address.

All new patients must have a health check with the nurse or healthcare assistant. This is an important part of registration enabling the practice to obtain medical information before your records arrive.

Please bring any medication you are currently taking as well as a urine sample to this first appointment (white topped sample bottles are available from reception).

Medical treatment is available from the date of registration. Please contact reception for further information.
